Vital Records (Birth & Death Certificates)

DOH-Sarasota provides certified copies of birth and death certificates to eligible individuals.  Stop by one of our locations or mail an application to the Ringling address.  A legal photo ID is required, along with the application. 

The cost for a birth certificate is $15.00 for the first certificate and $5.00 for each additional certificate purchased at the same time for the same individual.  

The cost for each death certificate is $10.00.  Death certificates are purchased in county where the death occurred. Sarasota County holds records from 2000-present. Note: If you require a death certificate for an individual who died prior to the year 2000, call 904-359-6900 x9000.
