Medical Examinations for Immigrants

Medical examinations are approved by the United States Citizenship and Immigration Services. The process requires two appointments.

Locations and Hours
To make an appointment, please call one of the following locations:

• William L. Little Health & Human Services Center (downtown Sarasota)
  Tuesdays: 7-10 am
  Adults only / by appointment only:  (941) 861-2859
• Child Health Center (17th St, Sarasota)
  Children only up to age 18 / by appointment only: (941) 861-1400

What to Bring
Please bring the following current (non-expired) government-issued identification and documents to your first immigration health appointment:

• Passport
• Form I-94 (if available)
• Vaccination records (translated to English)
